Purpose
To reduce the infant mortality, low birth weight, and SIDS rates in the African-American community. Provides prenatal outreach and care coordination, nursing case management, social support and parenting classes, and home visiting services. Offers support for women up to 12 months following delivery, as well as a Reclaiming Fatherhood Group and parenting support for fathers.

Who is eligible
Pregnant and parenting African-American women and their families, especially those at-risk of delivering a premature or low birth weight baby. Participation is voluntary; client confidentiality is respected.

  • Help with finding a doctor
  • Referrals to community health and social services
  • Assistance with Medi-Cal and insurance for kids
  • Public Health Nursing services
  • Home Visiting and care coordination services
  • Parenting classes and resources
  • Networking groups for pregnant and parenting women
  • Networking groups for expectant and parenting men
  • Case management support for fathers
  • Raising a Reader book bags
  • Free baby clothing shopping at Stork's Nest 

Community Mobilization
Encourages community awareness, support and advocacy for healthy African-American babies. Is supported by a Black Infant Health Advisory Group. Sponsors annual "Celebrate Healthy Babies Events". Collaborates with other programs such as the Prenatal to Three Program, Adolescent Family Life Project, March of Dimes, and other community health and social service providers.
