As a unit of the Health Department, the Health Policy and Planning unit priorities and goals are mechanisms for increasing the Health Department's ability to successfully address its broader initiatives: Healthy Communities San Mateo County; Youth Development; Community Capacity. The one-year and three-year milestones detailed below are progress measures, or next steps, for advancing the Health Department's initiatives.

A community where collaboration eliminates health disparities and improves health and well-being for all.

Mission Statement
Health Policy and Planning (HPP) increases the Health Department's ability to improve community health by addressing strategic health issues in partnership with the community. HPP carries out projects and initiatives aimed at long-term and systemic impact on health for all of San Mateo County.

Core Competencies
- Broad view of Health Department Mission across the seven divisions
- Commitment to partnering with communities in improving community health
- Understanding of the impact of root causes of health disparities on community health
- Ability to factor national, state and local trends and emerging research findings into priority setting
- Understanding of the strengths of approaching priority health issues through cross-sector collaboration
- Ability to use a wide range of data (e.g., qualitative, quantitative, academic and community findings) to inform program and policy considerations

Unit Priorities
- Educate and empower County residents, coalitions and networks to address priority health issues.
- Eliminate health disparities in partnership with County Departments and community in linguistically, ethnically and culturally appropriate ways.
- Research, analyze and inform innovative solutions to health problems and emerging health issues.

Unit Goals
- Mobilize community partnerships to address key health issues.
- Further develop and begin implementation of a comprehensive internal adn external plan for addressing and eliminating health disparities.
- Leverage strategic community health promotion activities as a resource in addressing key health issues.
- Provide technical assistance in the following areas: internal and external needs assessments; maximization of funding opportunities; curriculum development; and evaluation; to Health Department programs and community organizations in order to increase efficiency and effectiveness in addressing priorities.
- Institutionalize processes to "connect the dots" across divisions and community partnerships in order to facilitate internal and external collaboration.
