If you would like information or data from the San Mateo County Health Department, please fill out a Data Request Form. The form can be downloaded by clicking HERE.


Please fill out the form as thoroughly as possible and be specific about the information you are looking for. All fields are required for submission. Once completed, email (epidemiology@co.sanmateo.ca.us) or fax (650-573-2919) the form to the Epidemiology Unit. Your request will be processed as quickly as possible.

Automated Vital Statistics System (AVSS)
The Automated Vital Statistics System (AVSS) is an online health information system designed to automate birth and death certificates as well as a variety of other forms. Several revolutionary principles are incorporated into AVSS, resulting in a means to bridge the information gap between the public and private sectors.


Behavioral Risk Factor Surveillance System (BRFSS)

The BRFSS, the world's largest telephone survey, tracks health risks in the United States. Information from the survey is used to improve the health of the American people.


California Health Kids Survey (CHKS)
The Healthy Kids Survey is a comprehensive youth self-report data collection system that provides essential and reliable health risk assessment and resilience information. Targeted at grades 5-12, the HKS enables schools and communities to collect and analyze data regarding local youth health risks, assets, and behaviors. The survey can help schools and districts meet all new assessment requirements of the No Child Left Behind Act (NCLB) for Title IV Safe and Drug Free Schools and Communities by accurately identifying areas of strength and weakness. It is designed to be part of a comprehensive data-driven decision making process to help guide the development of more effective health, prevention, and youth development programs.


California Health Interview Survey (CHIS)
CHIS is an important source of information on health and access to health care services. CHIS is a telephone survey of adults, adolescents, and children from all parts of the state. The survey is conducted every two years. CHIS is the largest state health survey and one of the largest health surveys in the United States. CHIS gives health planners, policy makers, county governments, advocacy groups, and communities a detailed picture of the health and health care needs facing California's diverse population.


HIV/AIDS Reporting System (HARS)

The HIV/AIDS Reporting System (HARS) is a collection of computer programs and data files developed by the Division of HIV/AIDS at the CDC that simplifies the management and analysis of HIV and AIDS surveillance data.


Office of Statewide Health Planning and Development (OSHPD)

Every six months, California licensed hospitals are required to submit specific data on every patient discharged from their facility. Reported data includes patient demographic information, such as age, sex, county of residence, and race/ethnicity; diagnostic information; treatment information; disposition; total charges; and expected source of payment. Data are available beginning with the 1999 reporting period.


 
Tuberculosis Information Management (TIMS)
The Division of Tuberculosis Elimination (DTBE) at CDC is in the process of transitioning users of the TB Information Management System (TIMS) to the National Electronic Disease Surveillance System (NEDSS) Base System (NBS) or to Public Health Information Network (PHIN)- compatible systems for reporting tuberculosis surveillance data (from the Report of Verified Case of Tuberculosis) to CDC. This site provides current information regarding the transition.
The Tuberculosis Information Management System (TIMS) is a surveillance and case management software application used by TB control programs in all 50 states, the District of Columbia, and various U.S. reporting areas in the Pacific and Caribbean.


Youth Risk Behavior Surveillance System (YRBSS)
The YRBSS was developed in 1990 to monitor priority health risk behaviors that contribute markedly to the leading causes of death, disability, and social problems among youth and adults in the United States. These behaviors, often established during childhood and early adolescence, include:
- Tobacco Use
- Unhealthy Dietary Behaviors
- Inadequate Physical Activity
- Alcohol and Other Drug Use
- Sexual behaviors that contribute to unintended pregnancy, STDs/HIV
- Behaviors that contribute to unintentional injuries and violence
