At the Senior Care Center our team is your team. We can provide your primary care right here in our clinic or at your home if it's appropriate. We can even work with your existing doctor if you prefer.
We're here for you because we've chosen to care for the well-being of seniors. We take the time to listen, get to know each patient, and thoroughly explain treatment options and recommendations because we understand what you might be going through. And, we want to make it easier for seniors to get the care they need all in one convenient place.
Caring for Older People Is Our Specialty
Taking good care of seniors requires the type of special training and sensitivity you'll find only at the Senior Care Center. Unique in San Mateo County:
- We understand the medical and lifestyle challenges that exist when seniors have several ongoing health problems.
- We coordinate all aspects of your care with other team members so nothing is missed.
- We understand the common conditions that often affect seniors and how aging affects those conditions.
- We understand the effects that aging and other conditions have on physical and mental health as well as on a person's independence.
- We know the appropriate use of medication so we can prescribe the right mix and dosages.
- We evaluate and organize healthcare and social services to help seniors stay as independent and productive as possible—for as long as possible.
- We assist families and other caregivers as they face new challenges, decisions and responsibilities.
We Help Seniors Stay Active and Independent for a Happier Life
Your Senior Care Center team treats the whole person. We take time to look at all areas of a senior’s life to help him or her obtain the best physical, mental, social and lifestyle health possible. We make a complete evaluation and consider other things such as diet and nutrition, medication management, pain management, assisted living needs, family dynamics, and the need for emotional and other support services.
We take this holistic approach because we believe it's the best way to help seniors stay as active and independent as possible for a happier life.
Our Interdisciplinary Team Includes:
- Physicians specializing in geriatrics and internal medicine, and a nurse practitioner who act as the cornerstone of the team to provide the care you need.
- A social worker who helps you find benefits, housing, activities, home care, and other services to improve your quality of life.
- An occupational therapist who helps you stay as independent as possible by finding ways to help with day-to-day living.
- A psychologist and psychiatrist who understand mental and emotional needs and who can give seniors the tools to address them.
- Nurses who provide education and resources for a healthy lifestyle.
- A podiatrist and optometrist for a complete circle of care.
Getting to the Center
The Senior Care Center is located in the San Mateo Medical Center. It is convenient to public transportation including bus routes 295, 390, 391, and 397. It is located approximately one half mile west of El Camino Real (see map). For patients who need help with transportation, based on physician authorization, the Medical Center’s van transportation service is available.
Insurance
We accept most insurance plans (such as Blue Shield and Aetna), Medicare, Medi-Cal, some Medicare HMO plans, and the County's WELL Program. Please call us to confirm that we accept your insurance plan. If you are uninsured, we can help you determine if there are any programs for which you might qualify.
Making an Appointment
To schedule an appointment, please call (650) 573-2426 between 8 a.m. and 5 p.m. and someone on our staff will be happy to assist you. Appointments can also be made by a patient's primary care physician, a social service agency, family members or other caregivers (Download and print the Referral Form). Center hours are Monday through Friday from 8:00 a.m. to 5:00 p.m.
