Congratulations on taking the first step to participate in the HEAR2T Program!
As a County employee, retiree or spouse/partner, you are encouraged to complete an on-line Cardiovascular Health Assessment (CRA) to determine your risk of heart attack or stroke. The CRA health questionnaire is maintained by Stanford researchers on their secure Stanford Heart Network website. As a member of the Stanford Heart Network, our employees can now go to their website, log in, complete the CRA, and take advantage of its many educational features.
The CRA takes about 10 minutes to complete and can be done at work or at home. You are asked to input basic health information and answer lifestyle and medical history questions. Your results are analyzed and displayed on the screen. You can update the information at any time. Additionally, you are encouraged to re-take the questionnaire to update your information and re-assess your risk. To take the on-line CRA health questionnaire, click here. You will automatically be transferred to the Stanford Heart Network website.
Throughout the year, we offer HEAR2TCheck Clinics so that employees can get updated date blood pressure and cholesterol readings, and complete the CRA using laptop computers.For the current clinic schedule, click here.It isn't necessary nor mandatory that you participate in the HEAR2TCheck clinic to complete the CRA. In fact, sitting at your computer at work or at home and inputting your lab and test results from your most recent doctor's visit is just as good!
Your individual CRA results are displayed on the screen. You also have option of printing out your two-page report to give to your doctor. If you are determined to be at increased risk, you will be encouraged to have additional health tests, seek medical care or attend health improvement classes. You also may be invited to participate in the HEAR2T Counseling Program.
