AETNA: Simple Steps to a Healthier Life® - Not currently Offered -
(for Aetna Health Plan members only ~ employees, retirees, spouses / partners)

Simple Steps To A Healthier Life® is an interactive online health and wellness program that makes it easy for you to help improve or maintain your health in ways that fit your lifestyle. The program takes you step-by- step to help you achieve your health goals. You will be able to identify some of your health risks, receive tailored Health Reports, a personalized Action plan, and participate in recommended Healthy Living Programs.

  • Step 1: Assess Your Health: Take the Health Risk Assessment and determine your personal health needs and your readiness to change certain health behaviors. Review your tailored health reports to help you focus on what changes you can make to improve your chances of remaining healthy and productive.
  • Step 2: Take Action: Your personalized action plan provides a guided step-by-step action oriented approach to making positive health changes. You will have many topics to choose from including Healthier Diet, Get In Shape, Stress Relief and Healthy Aging. These multi-session programs are easy, fun, motivational and adaptable to your needs and interests. Weekly e-mail reminders will help you stay on track.
  • Step 3: Learn More: This section provides you with easy-to-find information and tools to help make informed health decisions. In addition, you will find links to local health and wellness resources.

 


BLUE SHIELD: Healthy Lifestyle Rewards Program
(for Blue Shield Health Plan Members ~ Employees only)

This online, interactive program rewards you when you adopt and maintain healthy lifestyle habits focusing on healthy eating, physical activity, stress management, and smoking cessation. Once enrolled, you’ll use a range of engaging tools, information, and advice to help you get started. Regular e-mails help keep you motivated, and because the program is online, you can access it confidentially anywhere, at any time.

With Healthy Lifestyle Rewards, you’ll be eligible to earn from $50 - $175 in cash or merchandise by participating in the program for a required minimum period of time. The earlier you sign up, the more time you have to earn the larger rewards.

Program Registration begins January 14, 2007.

Kaiser recently upgraded its website to include new online health improvement programs focusing on Nutrition, Weight Loss, Stress, and Smoking. Other new online services include online health support and personal health coaching.

KAISER: the P.H.A.S.E. Program (Preventing Heart Attacks   and Stroke Everyday)

P.H.A.S.E. focuses on improving the health status and health outcomes of County Kaiser members who have diabetes, kidney and/or heart disease. The Program goal is to improve health status and quality of life through aggressive medical management, medication compliance, and lifestyle changes.

Kaiser ~ Redwood City has reviewed electronic medical records and pharmacy information and has identified 239 employees, retirees and spouses/ partners who are eligible for this program.

During the course of the six-month program, participants have regular interactions with a Care Manager (Registered Nurse and/or Pharmacist), which can include patient education workshops, one-on-one visits and/or telephone appointment visits. Monthly 90-minute patient education workshops focus on relevant health topics including nutrition, exercise, smoking cessation, depression and general self-care issues.
