Home Visiting/Case Management
Eligibility Requirements:
• San Mateo County residence
• Eligible for Medi-Cal
• Pregnant woman or mother of child under age five
• Newborn child, or child up to age five if born outside San Mateo County

Extension of Eligibility:
Uninsured women or children under age five who are uninsured and ineligible for Medi-Cal but meet the following conditions:
• Pregnant women with mental health and/or alcohol and drug problems
• Parent with mental health and/or alcohol and drug problems who has a child under age five
• Child under age five with mental health problems

Nutrition
Each Registered Dietitian is a Certified Lactation Consultant and provides home visits/case management to Medi-Cal eligible women and children under five in San Mateo County. Upon referral and client approval, the Registered Dietitian will provide in-home nutrition/breastfeeding assessment and linkages to providers and resources throughout the County.

Parent Support: Free multi-lingual community-based groups
• Touchpoints Parent Support Groups (available to all San Mateo County Residents)
• Parenting Education Classes
Eligibility Requirements :
– San Mateo County Resident
– Parent or primary caregiver to child under age five
– Need for 14 week course of instruction, discussion, homework and practice

Mental Health Groups: Dual Diagnosis, Parent-Child Interaction, Parent-Child Emotional Regulation
Healthy Moms, Healthy Children Program: Parenting is a challenge for anyone. Sometimes it can be even harder because of your life experiences. If you have feelings that come up because of old wounds, a desire to learn how to manage these feelings in a safe, confidential place, hoped for support from others around the changes you and your baby will go through, then Healthy Moms, Healthy Children may be for you.
Eligibility Requirements:
– San Mateo County Resident
– Parent or primary caregiver to child under age five
– Mental Health Diagnosis

How to Make a Referral
Referrals can be made by doctors, nurses, health care workers, health and social service providers by simply calling (650) 573-2502 or (888) 840-0889, or by faxing (650) 341-0674. Self-referrals can also be made at any time.
The following referral information is required:
• Mother's Name, Address, Telephone Number
• Mother's Date of Birth
• Mother's MediCal Identification Number
• Mother's Delivery Date
• Area of Concern
